How To Complete a Client Services Agreement

Welcome to Ours! If you are new here, it's important to fill out your client service agreement so you can get started with your therapist.

Here's How:

1. Log on to our secure platform: https://secure.withours.com/
2. On the top right of your screen click the "account" button.
3. On the next screen that pops up, select "profile" on the top left-hand side.

4. Scroll down until you've reached where it says "informed consent at Ours". Select the button that says "click here" to review your intake paperwork. Your informed consent paperwork will populate for you and your partner to complete. You also will be able to come back to this same spot if you would like to review it at any time.
